Mighty Silverfist does not drop passive skill points book

I killed Mighty Silverfist two times, and I cannot get skill points drop.

My skill points show 8/8 (include all from act 1 and act 2). The map still show active Mighty Silverfist icon.

Even after waiting for a day with a refreshed map.

And Mektul The Forgemaster does not drop hammer for Oswald quest.
