Omen of Resurgence buggy

This is the 3rd time i die on hard core with this item in my inventory. i specifically buy it to give me a chance to survive.

yes , it IS activated there is a nice red ring around it so hard to mis and for the 3rd time now i have died without it triggering. wth is going on ?

Is it possible that you are never reaching below 25%? ie. getting one shotted and bypassing that range in between 0% and 25%? I don't know if it protects against that or not, I have no experience using the item before.

Only other thing I can think of is in POE1 you can only use one omen per zone. So if another one has triggered, it will prevent the second one from proccing.
