Perfect Exalt to T4 bug?

I captured my slamming a perfect exalt that resulted into t4. Is this a bug? I thought t1 is a guarantee for perfect exalt and there is no way that a perfect exalt can have a t4 outcome or am I wrong? Kindly see the video below. I was able to capture the whole thing. Did it possibly take the rune numbers in? I am so confuse.

Would you mind giving me the link to that information? I have perfect exalted a couple of times already and this is the first t4 outcome.
