Stuck in Failed Trial of the Sekehmas

On 3 separate occasions (different days) upon failing the trial I was unable to exit. I have attached screenshots of 2 of the 3 occasions, notice the button to exit is missing. I had to close the game and log back in. Oddly, I have failed between these occasions and have been able to exit without issue, so I'm guessing there is some underlying cause (death by something particular or buy loss of honor instead of by HP reaching zero) but I'm not sure what.
