Rake Herald of Blood Issue

So, I'm not certain what the actual issue is; if it's Rake that isn't applying bleed or if Herald of Blood isn't procing every time it should.

What's happening is I'll jump into a group of enemies and attack with Rake, which has 100% chance to apply bleed, and when enemies die they are not always exploding as Herald of Blood indicates they should.

Same issue here. To me it seems like Herald of Blood doesn't work when the targeted mob gets one-shot. I hope they will change this, as my Rake + Herald of Blood combo is now useless.
