Corrupted nexus not working

I completed the second corrupted nexus with 4 mods on a tier 7 waystone but the map did not spawn the final boss yet counts as completed. This led to the surrounding area not being cleansed either anyone able to clarify what happened or give some help to this matter?

I used an overseer precursor tablet and the corrupted nexus got a boss added to it. I cleared the map and killed the normal boss. No corrupted boss spawned.

The whole area has been cleansed, but I did not receive credit for completing the T4+ corrupted nexus completion.

I just completed a corrupted nexus that had an inherent boss mod. It wasn't under the influence of any towers. I cleared the map using a T15 waystone with 5 mods on it. After all rares were gone, I proceeded to the boss - which was the queen of filth. She barrel rolled my head immediately upon jumping out of the pool, so I used my only revive. I quickly killed her on the 2nd attempt but she didn't drop the skill book per the quest "Complete a Tier 4+ Corrupted Nexus"

The surrounding map nodes updated to cleansed, so I think it worked partially. The skill book should have dropped, but I'm guessing it failed to do so because the entire map resets on player death on a boss.
