omen of ameleration failed

TWice, I have died with an omen of ameleration (however that is spelled)
and when I revive, I have lost 10% xp and the omen is sstill in inventory and turned on.

This has happened to me twice now.
I have no pattern.

Slighly related. As others have said death penalty at high levels is unreasonable.
Also you Need to notify the player of what killed him.

mind you, toniht I was playing my minion witch with nearly 8K ES and got one shot by a trash mob, not rare of named.

Did you use another Omen in the zone? Only one Omen can be used per combat area.

ok, it might have been a second death. good to know.
