Omens and Thawing Charm

I have found that omen of amelioration doesn't trigger after the first time you have used one in a map. So if one triggers from your inventory from dying while in a map and you portal out, get a second one and activate it, the second omen will not trigger when if you die again in the same map. Also, omen of amelioration will trigger if you die while fighting a pinnacle boss if it is active in your inventory. I don't think it should do that since pinnacle bosses do not give exp or take away exp. Lastly, I have an issue where when I have a thawing charm equipped it says that I have 0/0 uses on the charm and using the well does not fix the issue so I currently can't use a thawing charm at all.

Charm thing is because you have a source of +Charges used (Ingenuity?)
