Stone citadel bug map failed bug

I beat the boss I got the weathered crisis fragment. map said it was complete I had 4 portals left.

When I left the boss room to finish killing the rares. I died to one of the rare's and it respawned me at the boss check point with a now undiscovered map.

It was now telling me that the boss wasnt dead despite his corpse still on the screen and nothing was left on the map to finish.

I used a new way stone to go get rid of the remaining portals and now the main maps says the stone citadel map was failed.
