Omen of Amelioration not working..

I just died twice in a juiced t16 Map. I had no other omens in my bag, the only omen I had and the only omen that was active was the omen of Amelioration. It did not get consumed, it did not show any flavor text for any omen when I died, and it happened multiple times on the oasis map that I was on.

What is going on with these omens? The whole point is to safeguard the massive waste of time that dying at high level can cause. What is even the point if they don't work? I want my time back because I played by the rules to preserve it and now its gone because your game isn't functioning as intended. Please address this.

The text at the bottom of the omen’s description says something along the lines of “only 1 omen can be triggered from combat per instance”. If you died twice, did you have 2 of the omens both activated in your inventory at the same time? That’s bugged out omens being triggered for me a few times.

Regardless, the omen should’ve only worked the first death with a 0% chance of working more than once in the same map
