Omen of amelioration bug

Please fix it, i'm lvl 96, it happened more than 10 time, with other omen like resurgence or amelioration only every AOE or One shot or stun lock death don't consume the omen. i'm not the only one who have this bug. thanks you

Not a bug. Only 1 omen can activate per area instance. your omen of resurgence is activating before you die so the omen of amelioration can no longer activate when you die.
