Character freeze/map reset

So I was going through a tier 16 Sinking Spire(tower) with a kalguuran expedition and Breach modifiers, I was on first floor and just finished clearing kalguuran expedition and started moving towards the next door but my character froze in place with a motion blur. Confused I tried closing poe2 and upon opening it again, I was back at hideout and my Sinking Spire had reset now with no modifiers.
