Reforge Bench is absent

Hello. After I died on hordcore on my both characters with nicknames ParenBloodPerturbationX and ParenWithcBithc I do not have access to reforging bench on softcore.

I tried to slain Molten Vault Boss on both difficulties but he does not drop quest item to aquire reforging bench. Also I tried to run through Drawned City and check it there is an NPC giving that quest. Nothing. Icould not find anything

How can I get it fixed?
