Technically not "new" but new to me.

While closing a corrupted Nexus using a tier 14 waystone with 4 explicit modifiers, I was killed suddenly. I had 3 attempts on the stone and succeeded with no further deaths.
At the end I received ground drop rewards but no atlas points.

This has also happened to me on unique maps twice with no deaths at all.
I would like to be able to improve my Atlas, and feel this is absolutely game breaking.
This should have been fixed at first discovery.
