Simulacrum bug, can't proceed and Entity can't be clicked

I got stuck in a T2 (100 splinter) simulacrum. I killed the waves of wave 9, no loot dropped and the boss is still in the area grunting but should be dead. I can portal out and into the area which made the "Strangely Familiar Entity" appear but I can't click it. Here is a short video showing the state:

Tried again just now and it again got stuck. Can't proceed to wave 14. Sais 29 monsters remaining but there are none.
