Inscribed Ultimatum (level 42) missing

I used an Inscribed Ultimatum (level 42), but when the entrance opened there was a prompt to press a key. However, I couldn’t enter the door no matter how many times I pressed it. Then I tried exiting and re-entering, but the door disappeared and my Inscribed Ultimatum was gone as well. Is there any way to get it back?
