Dying despite pausing the game in a map.

I don't know how common this is, but I have experienced frequent and multiple time the game "lags" out and the screen freezes then goes black and cogs show up spinning as it tries to reconnect. During these instances I hit escape as soon as it happens to avoid the game continuing to play in the background as I do not trust the auto-pause. During my breach in my map this occurred I hit escape assuming falsely I would be safe and that the game would pause as we have been informed that it would. I however died during the black screen while the cogs were turning despite the game's pause menu being opened... What the hell is up with that?
