Hammer of Kamasa not dropping on pc

tried doing Molten Vault on another character that I was leveling and the Hammer of Kamasa wouldn't drop from the boss.

was playing a Pathfinder using a crossbow at the time and had already opened up the Temple of Kopec (don't know if that area opening up is blocking progress in this side quest or not but figured its best to report it just in case). did the Molten Vault boss twice now with no progress
