3 Mushrooms quest bug - only 2 types fall but quest completes completes

Playing solo:

Picked up the 1st mushroom (green) and character announces "That's Three"
Turned it in to cauldron. Nothing happens
Then blue and 2x Green drops
Sorry, I didn't check inventory to see if I actually had two green in there.
I don't think I did though, since clicking on one made the other disappear.
Turn those in
quest Complete and never even saw a red mushroom

Minor bug, but an odd one

i remember that i dropped all 3 different colored mushrooms with no problem, maybe an isolate bug?
