Kanu and the First Herald doesn't appear

I reached this fight and died at the same time as the boss. When I revived, the boss wasn't there, and I couldn't continue. I tried restarting the instance, but the boss text appeared, but it disappeared, and I couldn't progress.
How can I fix this issue?
