Unable to complete Stones of Serle on Demon Form Witch

After activating the stones and killing the boss, Una comes but the quest does not complete. I can click on Una but the options are grayed out.

Other demon form bugs:
In act 4, when in demon form, getting off the little row boat causes the character to be frozen until respawn.

In Well of Souls when you reveal an item, the character also gets stuck.
