100% Mana before Life + CI Deaths + Eldrich Battery

Hello, I am running 100% Mana before life + Chaos Inoculation + Eldrich Battery,
I died a few times instantly and when I still have mana, sometimes full/allmoust full mana (about 7k tons of regen etc) so somehow something gets to my 1 HP and NO I AM NOT USING A FLASK that uses life. Is there any debuff that affect damage redirection or something? or something is just bugged? this dont happen to often... so its something "rare" like 1 in 70 maps maybe... kinda sucks cus I am at lvl 97 and death penalty hurts a lot... any ideias?

I am trying 104% mana before life for a while just to check for some weird round up or something...

something like Heavy Buffer or anything that allow damage to bypas ES?

I have no ES, pure mana from Eldrich Battery

So I went 104% mana before life and this didnt happen again yet, it may be some kind of isue with 100% going 99% somehow... if it happens again I will take printscreen and post it
