Black Cathedral will not create new instance

I died the same time beating the pinnacle boss in the Black Cathedral, after respawning, no loot was on the ground and the area will not create a new instance.

new istance: go back to caravan, use waypoint to well of souls, approach black cathedral entrance and hold the button to create new istance.
