4th Ascendancy hard crashes

I'm experiencing crashes during the 4th Ascendancy trial while playing as a Druid in wolf form. I've been attempting to respec to test out the other Ascendancy, but after six attempts, I keep encountering hard crashes either midway through or when entering the last boss room.

I've verified game files.
I've tried both Vulkan and DirectX12.
