Trial of Sekhemas: Minor Boon "Silver Chalice" not functioning

Doing a Sekhema run and purchased the minor boon 'Silver Chalice'. I then purchased an additional minor boon 'Fright Mask' and it was not upgraded to a major boon. Silver Chalice was not consumed after purchasing the second boon from the merchant.

This is the first time I have encountered this, and the only thing I can identify doing differently is purchasing the boons in very quick succession.

Edit: Silver Chalice was consumed after activating a shrine that provided a boon in the end of one of the challenge rooms. The problem seems to be related to the merchant.
