Help, citadel quest not appearing for any of my characters.

Not sure what to do Im 100s of hrs into endgame and still don't have the quest for citadels. I think its also affecting the spawn of them. Im well over 10 screens away in several directions and only found 1. i tried forcing the quest by buying the fragments it didnt help.
