No accendency points from Trial of Sekhemas

I completed the lvl 60 trial of sekhemas and did no receive any accendency points despite the quest item saying I would. I thought it was a bug and went to create a new instance to try again, died, and the quest item was destroyed. I'd like to know how this can be resolved so I can earn the accendency points.

How many points do you have, and which floor did you make it to?
For example, if you have 4 points, the Altar at the end of the second floor can be used to respec to a different Ascendancy--but for points 5+6, you need to use the Altar at the end of the third floor.

I finished trial, I had and still have 4 ascendency points. I did not attempt to respec my ascendency. I would be perfectly happy to just do the trial again but my quest item no longer exists, and I do not have any more ascendency points.

Then you must obtain another 60+ Barya to try again. You must defeat the boss of the third floor for points 5 and 6.

Sometimes people finish the first floor of the trial, don't notice the door in the back leading to the second floor, and leave.

Which bosses did you fight?

The first floor's boss is Rattlecage, the big rock using volcanic-style skills.
The second floor has two guardians, one fire-themed and the other ice-themed.
The third floor's boss is a poisonous scorpian, that can dive underground and create quicksand.
