#2353785297 Grand Adjudicator Nexus No Atlas Points After Revive

I cleared a nexus, and fought the grand adjudicator. Upon his death, and on-death effect (I assume, not able to tell for certain) killed me. The corruption boss began spawning, but stopped once I died.

After reviving I received the "corruption cleansed" notification, and surrounding area was cleansed. However, I didn't get any atlas points (T12 map quest), and neither boss was able to be reattempted.

This map had 2 revives.
