0.4 PC constant and consistent crashing on Vulkan and DX12

Hello,
There is a bug that is causing full loss of display out from GPU and requiring a full system restart to get the display back. This is happening on both Vulkan and DX12. I believe that this has been happening all league, but has become extremely noticeable within the last week or two. This has reached the point of being unable to play the game whatsoever with crashes happening multiple times in a single map. I have updated drivers, DDU'd drivers, and rolled back drivers and none of them seem to fix this issue. I have logs for both DX12 and Vulkan and can reproduce consistently if needed.
