Bugged Spirit and Passive Points

Upon reaching the interlude on a second character, and defeating the required bosses (Lythara & Oswin) I noticed that when they dropped their respective loot (Gemcrust skull and Passive Book) the drops weren't added to my inventory upon picking them up. On subsequent attempts, the bosses did not drop the Passive Book or the Gemcrust Skull at all, leaving my character with 40 missing spirit and 2 missing passive points. The bosses show as "uncomplete" on the map still, with seemingly no way to "complete" them.

Do you happen to be a titan and have the items in your extra inventory?
