Towers only allowing 1 attempt regardless of waystone

I used a grand project on a tower attached to a T15 waystone with 2 revive attempts. I also used a second irradiated precursor tablet. When I opened the map for the tower, it only gave me one portal. I actually died to being stunlocked and indeed, I was removed from the map and it failed, losing the ability to finish the map and losing my grand project tablet. This happened two towers in a row on separate towers. I then used a T1 waystone on the failed map and it opened all 5 portals. Not really sure what is going on. This has happened in the past, and I thought maybe I was just not paying attention and put a 0 revive waystone, but I specifically tested it this time and got 0 revives on 2 revive waystone.
