Concept 1
Front: small Path of Exile logo on the side
Back: visual of Sirus hovering over the watcher, seen from below, with the caption "How boring and small"

Concept 2
Front: small Path of Exile logo on the side
Back: portrait of the Maven in a mist with the caption "The price piece comes to join" on top and "my collection" in slightly bigger characters on the bottom.

Sorry, but I really think the rewards should be monetary...
If you are getting an artist work from this, then the rewards should be for chosen people few thousand $ each.
Otherwise you are trying to use your community and not reward it.
