Not getting Reward from Ignagduk, The Bog Witch

Killed her using molten strike and shield charge, inventory was full at the time but when I clicked on gemrot skull it just completely disappeared along with the spear. When interacting with Servi afterwards she acted as though the quest was completed and gave her rewards (charm and orb). After that I attempted 3 more times in new instance, closing the game, trying with a friend. Waiting for old instance to close as well.

Im just dumb. It was in my extra invetory stash as a titan.
