Omen of Amelioration bug

If there are more than two of these in the stash, sometimes it doesn't work. This bug has been around for a while, but I haven't bothered to report it. I just keep one in my stash, and if I die, I go back to pick up another one.

This morning, I was too rushed to finish a map boss before heading out of the house, so I didn't have time to follow my usual routine. This has punished me severely. I died a second time and lost many hours of grinding.

This issue is still happening. It occurs multiple times daily.
It appears as though the omen can only be used once per map. In addition all associated map bonuses seem to disappear, while all nerfs stay in effect.
If the omen has a 1 per map item restriction then this needs to be clearly defined.
If the penalties i.e. resistance nerfs, difficulty, etc. are in place x.p. bonuses also need to remain. If a player loses all benefits and runs the risk of a setback they should know whether to bail.

I have lost way too much xp from this. Players should never be punished for developer error or short sights, and the game has a p.v.p. bully culture feel right now.
It isn't fun, and the point of the game is entertainment.
